Home Care Aide
personal care and household management activities under professional nursing supervision.
Responsible for adherence to a care plan that may include helping patient with personal
hygiene, preparing meals, running errands, accompanying patient to the physician's office,
light housekeeping, helping patient with speech, language or physical therapy programs.
Must practice universal precautions and infection control techniques. Requires completion
of 77 hour training course, passing both written and practical competency exam and
certification from the NJ State Board of Nursing. Requires completion of 12 hours of
in-service education each year.

clinical case management to home care clients while ensuring the best possible patient
care. Performs comprehensive assessments on new cases. Works with the client, family and
home care staff to draft and implement a plan of care. Supervises and evaluates home care
workers in the field. Assists with home care staff training and continuing education.
||Formulates care plans for patients not under a medically directed plan of care and
supervises homemaker-home health aide personnel. Visit homemakers in the field regularly
and reports on case progress and personnel service. Coordinate various grant programs run
by the agency. Assist in supervisory activities in child welfare cases.
